The material on this page will help you to apply for Bid, Performance, Payment, Maintenance, and Supply bonds.

As surety is a credit relationship, applying for a bond is similar to applying for a bank loan.

Sufficient time is needed to carefully consider all aspects of your submission.  The more information you can provide with this submission, the quicker a decision can be made.  Analysis can be very time-consuming, particularly if all items are not included in your submission.

Please provide us with the following information:

1.     Contractor Questionnaire (Download Here)

Please answer all questions completely.

2.     Business Financial Statements

Provide copies of the company’s fiscal year-end financial statements for the last three years. These statements must be prepared by an independent CPA firm.  All year-end statements must include Cover Letter from accountant on their letterhead, Balance Sheet, Income Statement, Statement of Cash Flow, and Notes & Schedules.

3.     Personal Financial Statements (Download Here)

All owners must provide a Personal Financial Statement.

4.     Resumes of Experience

All owners and key employees

5.     Work-on-Hand Schedule (Download Here)

Please provide a breakdown of ALL current jobs – both bonded and unbounded.

6.     Banking Information

Evidence of borrowing capacity and/or line of credit is required from your bank.

7.     References

Please provide letters of reference from suppliers and previous jobs, if available.

  • For bid bond requests, please provide the bid invitation/advertisement and any required bond form
  • For final bond requests, please provide a copy of the contract, bond requirement information and any required bond form
